Ionizing Radiation Dosimetry

Accreditation Valid From: April 1, 2008 Through: March 31, 2009

This facility has been evaluated and deemed competent to
process the radiation dosimeters listed below employing the
Panasonic automatic readers model UD-7900M for the
whole-body dosimeters and the Harshaw 6600 Automatic TLD
Reader for the EXT-RAD & DXT-RAD extremity dosimeters.

This facility is accredited to process the following
dosimeters by virtue of actual demonstration of compliance
with ANSI HPS N13.11-2001 through whole body testing.

Panasonic TLD model UD802AT in ISA model 820-C hanger for
ANSI-HPS-N13.11-2001 categories IB, IC, IIA, IIIA, IVA, VAA,
and VICA.

Panasonic TLD model UD802AT in ISA model 811 neutron hanger
for ANSI-HPS-N13.11-2001 categories IIA, IVA, VICA.

Panasonic TLD model UD802AT in UD874 hanger for ANSI
N13.11-2001 categories IB, IC, IIA, IIIA, IVA, VAA, VICA.

Thermo Electron EPD-2 (electronic dosimeter) for
ANSI-HPS-N13.11-2001 categories IIA, IIB, IIIA, IVA, VAA.

Thermo Electron EPD-N2 for ANSI N13.11-2001 categories IA,
IIA, IVA, VICA.

By actual demonstration of compliance with ANSI HPS
N13.11-2001, the deployable Field Laboratory to Assess
Radiation Exposures (FLARE) has been evaluated and deemed
competent to process the radiation dosimeters listed below.

Thermo Electron EPD-2 (electronic dosimeter) for ANSI HPS
N13.11-2001 categories IIA, IIB, IIIA, VIA, VAA

Thermo Electron EPD N-2 (electronic dosimeter) for ANSI HPS
N13.11-2001 categories IA, IIA, IVA, VICA

This facility is accredited to process the following
extremity dosimeters by actual demonstration of compliance
with ANSI HPS N13.32-1995 through testing.

Harshaw EXT-RAD extremity TLD-100 chip in a finger ring
strap for ANSI HPS N13.32-1995 categories IVA, VA, and VIIA
