Accreditation Valid From: October 1, 2009 Through: September 30, 2010

This facility has been evaluated and deemed competent to
process the radiation dosimeters listed below through
employing a Panasonic automatic reader models UD710A or
UD705A.

This facility is accredited to process the following
dosimeters by virtue of actual demonstration of compliance
with ANSI HPS-N13.11-2001 through testing.

Panasonic TLD Model UD802 AS/AT in an ISA Model 830 holder
or equivalent for ANSI-N13.11-2001 categories IB, IIB, IVB,
VCB, VICB.

Panasonic TLD Model UD802 AS/AT Neutron Pack in a Model ISA
830/ISA 810 holder or equivalent for ANSI-N13.11-2001
category VICB.
